BPS COVID-19 Vaccine Clinic REMINDER for Students ages 12 and older 

Dear Belmont Families,

We are partnering with Pelmeds Pharmacy to offer the 2 dose Pfizer Covid-19 vaccination series to our students ages 12 and up.

First Dose Clinics

Date: Monday, May 24, 2021

  • Chenery vaccine clinic hours 8 am - 2 pm
  • Belmont High School vaccine clinic hours 8 am - 12 pm

IMPORTANT THINGS TO KNOW - There is still time to register

Pelmeds Pharmacy has indicated that there will be plenty of vaccines for everyone. Additional appointments will be added if needed should all of the time slots fill up. 

If you experience any issues with the registration process please email info@pelmeds.com  to resolve any issues. 

After registering you will receive a link with a username and password. If you click on My Vaccine in the email and Login you will see a screen with all of the info that you input. Click Next and then you should be able to PRINT the consent form and SIGN

Please make sure to print and sign the consent form (in some cases this may be on a second page) and have your student bring it to school on the day of the clinic. 

A Parent or Guardian signature is required for any students under the age of 18. Parents/guardians do NOT need to be present, but we do need the student to bring the signed consent form to school on the day of their appointment.

Your student will receive a vaccination card after they receive their vaccination. Please keep this in a safe place. Your student will need to bring it to school on the day they receive their second dose in June. 

All students will be vaccinated free of charge regardless of insurance coverage. If insured, please enter insurance information in the online registration. Once registered online there is no need to bring an insurance card to the clinic.

A follow-up clinic for second doses will be held on June 14, 2021. An additional registration link will be provided to register for the second dose at a later time. Students who have already received their first dose of the Pfizer series at another location are welcome to register for either date to receive their second shot as long as there have been at least 3 weeks between doses. Please remember to send the vaccination card from the previous vaccination and consent form to the school with your student.
