REMINDER Second COVID-19 Vaccine Clinic for Students Ages 12 and Older

REMINDER.....BPS COVID-19 Vaccine Clinic for Students Ages 12 and Older 

Dear Belmont Families,

We are happy to announce that we are partnering again with Pelmeds Pharmacy to offer a second clinic for the Pfizer COVID-19 vaccination to our students ages 12 and up. Their team of vaccinators will come to our schools to administer the vaccine.

When:           Monday, June 14, 2021

Where:         Chenery Middle School 8:00 am - 2:00 pm

                      Belmont High School 8:00 am - 11:00 am

If you are a first-time user of the PelMeds system: After registering, you will receive a link with a username and password. Click on My Vaccine in the email and Login, and you will be brought to a screen where you will input your student’s information. Click Next and then Print and complete the consent form.

If you are a second-time user of the PelMeds system: You will schedule your student’s appointment using the appropriate link below. You will receive a confirmation email for the scheduled appointment. You must print and complete the consent form.

Your student must have the following documents with them in order to receive the vaccine:

  1. A completed and SIGNED Consent Form. A parent/guardian signature is required for any student under the age of 18.

  2. If this is your student’s second dose, they must bring their COVID-19 Vaccination Record Card. 

PelMeds Pharmacy has indicated that there will be plenty of vaccines for everyone. Additional appointments will be added should all of the time slots fill up. 

All students will be vaccinated free of charge, regardless of insurance coverage. If insured, please enter insurance information in the online registration. Once registered online there is no need to bring an insurance card to the clinic. 

Students who received their first dose of the Pfizer series at another location are welcome to register to receive their second dose at this clinic as long as it has been at least 3 weeks since their first dose but no longer than 42 days (per DPH). Please remember to send the vaccination card from the previous vaccination and a signed consent form to the school with your student.

Students are welcome to register to receive their first dose at this clinic and will need to follow up elsewhere for their second dose. Students will receive a vaccination card when they receive their vaccine. Please keep it in a safe place.
