Please find the electronic version of the order form for Belmont Middle School Picture Day scheduled for Friday, October 4th 2024.  All the instructions are on the form, as well as the pre-order school code. 

ONLINE ORDERS STRONGLY RECOMMENDED.  No forms need to be returned by parents.

The pre-order option will be available through Wednesday, October 9th at 7:00am. All print orders will be mailed directly to the home addresses provided at checkout. If parents do not order by the pre-order deadline, their child will receive a NO BUY proof card with instructions on how they can order at that point. The NO BUY proof cards will be sent to the school for distribution.

IMPORTANT**Please be advised that orders placed after the deadline are subject to higher package pricing and shipping/handling fees. The package options are different and some items may not be available.

Team Pictures (6) will also be photographed on Picture Day.  The option for parents to purchase the TEAM PICTURES will be offered separately at a later date.
