Employees Under Age 18
Welcome to the Belmont Public Schools. Below you will find all of the required forms and information you will need to begin your employment with BPS. Please contact the Human Resources Department with any questions.
New Employment – Required Forms
All new employees under 18 years of age must apply for a work permit and complete employment forms for Belmont Public Schools. Please follow instructions in both sections below.
Work Permit
If you are under 18 years of age you must obtain a work permit before starting a new job. Please note that a Physicians Certificate of Health and School Record Form are required only of 14 and 15 year-olds. See the following links for more details:
Complete all sections of the form(s) below and submit to your town's school district, who will then issue you a work permit. Residents of Belmont should submit the forms in-person at the Belmont High School Main Office, along with valid proof of date of birth and residence address (e.g., driver's license). If you are not enrolled in the Belmont Public Schools, you will also need to bring a School Record Form completed by your school (14 and 15 year olds) or proof you have completed the 6th grade (16 and 17 year olds).

Employment Forms
Please click on each link below to download, print, and complete each form. Forms are also available in the Human Resources office. Completed paperwork must be submitted in-person to the Human Resources Department, along with a valid photo ID and proof of authorization to work in the US (See I-9 form instructions for allowable forms of identification). The Human Resources Department is located in the School Administration at 644 Pleasant Street, Belmont.

CORI Release - Minor Under 18 - Parent/Guardian must complete this form
Direct Deposit Authorization Form
must also attach an official printout from your bank showing your name, routing number, and checking account number (e.g., voided check, or bank's direct deposit form available from your bank teller or online portal)
the account must be in the employee's name, or the employee must be a joint account holder with a parent
Required Training
Training consists of online documents which you can watch from your home computer. New BPS hires under the age of 18 must complete all training modules below (step 1) and verify with their digital signature and that of their parent/guardian (step 2), within 30 days of your date of hire.

2. When you have read and understood all of the documents and any questions have been answered, click here to provide your digital signature acknowledging that, as a condition of your employment in the Belmont Public Schools, you have completed all of the required training for this year. Your legal parent or guardian must also sign the Google form, confirming that you have completed all of the required training.
